Discharge Summary Header Section<\/h3><p>Discharge Summary should have a top Header Section. This usually has the following information:<\/p><ul class=\"wp-block-list\"><li>Patient UHID (Unique Hospital ID) and Name<\/li>\r\n\r\n<li>Page Age and Gender<\/li>\r\n\r\n<li>Patient Admission Number<\/li>\r\n\r\n<li>Patient Admitted on Date and Time<\/li>\r\n\r\n<li>Patient Address<\/li>\r\n\r\n<li>Patient\u2019s Discharge Status: Discharge Status will capture if the patient was Discharge Normally \/ Discharged against medical advice \/ Transferred to Another Hospital etc.<\/li><\/ul><h3 class=\"wp-block-heading\">2. Treating Consultant \/ Surgeon Name and Practice<\/h3><p>Discharge Summary should mention the Name of the Doctor under whom the patient is admitted for treatment. The specialty of the Doctor should be mentioned as well.<\/p><h3 class=\"wp-block-heading\">3. Admission Details Section<\/h3><p>In Admission Details Section, following important details are captured:<\/p><ul class=\"wp-block-list\"><li>Provisional Diagnosis at the time of Admission, if any<\/li>\r\n\r\n<li>Presenting Complaints with Duration and the Reason for Admission<\/li>\r\n\r\n<li>Key Findings on Physical Examination at the time of Admission including the Vital Signs<\/li>\r\n\r\n<li>General Findings at the time of Admission related to Eyes, Neck, Throat, Head, Heart etc.<\/li><\/ul><h3 class=\"wp-block-heading\">4. Hospitalization Details Section<\/h3><p>This section captures the medical information related to patient during the stay in Hospital. It usually includes below sections:<\/p><ul class=\"wp-block-list\"><li>Summary of Key Investigations during Hospitalization<\/li>\r\n\r\n<li>Course in the Hospital including complications, if any<\/li>\r\n\r\n<li>Treatment Given during Hospitalization<\/li><\/ul><h3 class=\"wp-block-heading\">5. Surgery Details Section<\/h3><p>If the patient has undergone a Surgery, this section should be present in the Discharge Summary. This section typically includes following details:<\/p><ul class=\"wp-block-list\"><li>Surgery Notes<\/li>\r\n\r\n<li>Name of Anesthetist<\/li>\r\n\r\n<li>Type of Anesthesia<\/li><\/ul><p>If no Surgical Procedure was carried out on the patient, this section can be omitted from Discharge Summary.<\/p><h3 class=\"wp-block-heading\">6. Discharge Details Section<\/h3><p>Discharge Details section captures details at the time of Discharge. These include:<\/p><ul class=\"wp-block-list\"><li>Discharge Status: Indicates whether the patient was stable at the time of Discharge or whether the Discharge was done against medical advice etc.<\/li>\r\n\r\n<li>Final Diagnosis at the time of Discharge<\/li>\r\n\r\n<li>Prescription on Discharge: Includes the medicine course to be adhered to by the patient after discharge.<\/li>\r\n\r\n<li>Advice on Discharge: General advice given to the patient. For e.g: Bed Rest for few days or Physiotherapy etc.<\/li>\r\n\r\n<li>Follow-up Date: In case a follow-up is required, mention the date for the next follow-up.<\/li>\r\n\r\n<li>Emergency Contact Information: Mention the details where patient can contact in case of any emergency.<\/li><\/ul><h3 class=\"wp-block-heading\">7. Signoff Section<\/h3><p>This section includes the Signatures of<\/p><ol class=\"wp-block-list\"><li>Treating Consultant or Authorized Team Doctor<\/li>\r\n\r\n<li>Patient or Attendant<\/li><\/ol><p>While signing off the Discharge Summary, make sure the document is thoroughly reviewed by the Doctors and then signed.<\/p><p>If you are a patient, make sure you have clearly understood what is written in the Document.
